Our paperwork is only the minimal amount that we need. We do not aim to drown anyone in paperwork when they arrive in our office. This cuts down on the writing time, and lets us get right to what matters most….helping you! If you want to save even more time, see below for a chance to complete your paperwork ahead of time.

New patient intake paperwork can be downloaded, completed and brought with you to your first appointment. This will allow you to spend less time filling out paperwork in our office, and more time getting you on the road to your comeback.
